Job Description: A Hospital Pharmacist manages medication therapy by reviewing physician orders, preparing and dispensing medications, and providing clinical expertise to multidisciplinary teams. Key duties include monitoring patient drug therapies for safety, counseling patients, supervising pharmacy technicians, and ensuring compliance with drug laws. They play a crucial role in improving patient outcomes through clinical rounds and medication reconciliation. Key Responsibilities

- Medication Dispensing & Compounding: Reviewing, interpreting, and dispensing medication orders accurately, including preparing sterile solutions.
- Clinical Surveillance: Monitoring drug therapies for potential incompatibilities, side effects, and therapeutic effectiveness.
- Collaborative Care: Advising doctors and nurses on drug selection, dosage, and administration routes, particularly in acute, ICU, or emergency settings.
- Patient Counseling: Educating patients, especially upon discharge, about their medication regimens.
- Inventory Control: Managing the supply, storage,



and security of pharmaceuticals, including controlled substances.
- Regulatory Compliance: Ensuring compliance with state/federal laws and hospital policies. Required Skills and Qualifications

- Education: PharmD (Doctor of Pharmacy) degree from an accredited college.
- Licensure: Valid pharmacist license to practice in the state.
- Clinical Knowledge: Strong understanding of pharmacology, therapeutics, and drug interactions.
- Communication: Excellent interpersonal skills for communicating with healthcare teams and patients.
