Job Title: Medical Coder - HCC

Location: Hyderabad

Job Summary

We are seeking experienced HCC Coders to join our team in Hyderabad. The ideal candidate will have a robust foundation in medical coding, excellent attention to detail, and a commitment to coding accuracy, compliance, and continuous improvement.

Responsibilities
Review medical records to identify and validate patient diagnoses, conditions, and treatments.
Assign appropriate ICD-10-CM diagnosis codes based on clinical documentation.
Map documented diagnoses to Hierarchical Condition Categories (HCC).
Ensure coding accuracy, consistency, and compliance with applicable coding guidelines.
Review and audit medical records and documentation for coding accuracy and compliance.
Identify documentation gaps and provide appropriate feedback to physicians and healthcare providers.
Support clinical documentation improvement initiatives.
Educate and guide team members on clinical documentation, coding guidelines, and HCC requirements.




Maintain productivity and quality standards while meeting established turnaround times.
Stay updated on changes to coding guidelines, HCC models, and healthcare regulations.

Requirements:
6 months to 4 years of relevant medical coding experience.
Experience in HCC coding / Risk Adjustment coding is preferred.
HCC Coding Certification is preferred but not mandatory.
Candidates with a strong medical coding background and willingness to learn HCC coding will also be considered.
Robust knowledge and practical experience in medical coding.
Good understanding of ICD-10-CM, CPT, HCPCS, and HEDIS Category II codes.
Knowledge of HCC coding and risk adjustment concepts.
Solid attention to detail and ability to interpret clinical documentation accurately.
Ability to identify coding discrepancies and documentation gaps.
Strong analytical, communication, and problem-solving skills.
Commitment to compliance, quality, accuracy, and continuous improvement.
