Roles & Responsibilities
- Serving as an advocate for patients and advising members of the medical care team about patients’ needs
- Conducting discharge counseling to ensure patients have the proper support, accommodations, supplies and equipment when they return home
- Assisting patients as they secure continuing care, such as home healthcare or in-patient rehabilitation therapy
- Referring patients to government agencies and nonprofit organizations that can assist with homelessness, food insecurity, unemployment or lack of transportation
- Coordinating care between medical providers for complex cases
- Helping patients explore medical financing options and prepare advanced directives
- Providing crisis intervention support through counseling
Eligibility Criteria
- Master's in Social Work:
- Minimum 1-2 Years of Experience in related field
