As we continue to grow, we are setting up a Remote Care Coordination Hub in India to support our UK operations in rostering, scheduling, compliance, and care monitoring. We are looking for an experienced Care Coordinator to join our remote team in Bengaluru (or nearby regions).

This role offers a chance to be part of an international healthcare group, with the possibility of travel to the UK for training and in office collaboration.

Role Overview

We are looking for a detail-oriented and proactive Care Coordination Associate to support care coordination for our UK operations. This role involves managing care plans, handling communication with clients and carers, and responding to care alerts through digital platforms.



The ideal candidate will possess strong analytical abilities, excellent communication skills, and the ability to work effectively with UK-based stakeholders.

*Key Responsibilities*

Care Plan Management
- Create and update care plans based on inputs from the UK clinical or operational team.
- Ensure accuracy, completeness, and timely updates of care documentation.
- Review care needs assessments and translate them into actionable care plans.

Client & Carer Communication
- Attend and manage inbound/outbound calls with clients and carers professionally.
- Address queries, clarify care instructions, and escalate concerns where necessary.
- Maintain a high standard of empathy and service quality during all interactions.

Care Alert Handling
- Monitor and respond promptly to care alerts received
