The Wellness Coordinator will be responsible for coordinating the overall psychosocial, emotional, and social well-being of patients, particularly elderly and long-term care patients. The role involves understanding patients’ and families’ needs, coordinating with clinical and support teams, facilitating patient engagement activities, and ensuring a supportive care environment.

Key Responsibilities1. Patient & Family Support

- Conduct psychosocial assessments of patients and identify emotional, social, family, and practical needs.
- Provide counselling, emotional support, and appropriate guidance to patients and their families.
- Support patients in adjusting to hospitalization, long-term care, rehabilitation, or changes in functional ability.
- Identify patients requiring additional psychological, social, or community support and coordinate appropriate referrals.
- Maintain regular communication with family members regarding non-clinical patient concerns.

2. Wellness & Engagement Activities

- Plan and coordinate wellness, recreational, therapeutic, and social engagement activities for patients.
- Organize activities such as games, music, art, celebrations, group interactions, and cognitive/social engagement programs.
- Encourage patient participation based on individual interests, abilities, and clinical condition.
- Coordinate special events and patient-centered programs within the hospital.

3. Care Coordination

- Work closely with doctors, nurses, physiotherapists, psychologists, dietitians, occupational therapists, and other departments to address patient needs.
- Participate in multidisciplinary discussions where required.
- Assist in discharge planning, particularly for patients requiring family, caregiver, rehabilitation, or community support.
- Coordinate with external agencies, NGOs, community resources, and support services when required.

4. Geriatric & Psychosocial Support

- Understand the specific social and emotional needs of elderly patients.




- Support patients experiencing loneliness, social isolation, adjustment difficulties, or family-related concerns.
- Promote dignity, independence, social interaction, and quality of life.
- Assist caregivers and families in understanding the emotional and social aspects of elderly care.

5. Documentation & Reporting

- Maintain individual patient psychosocial assessment and intervention records.
- Document counselling sessions, family interactions, referrals, and wellness activities.
- Prepare periodic reports on patient engagement and wellness programs.
- Maintain confidentiality of patient and family information at all times.

6. Patient Experience

- Act as a patient-support point of contact for non-clinical concerns.
- Identify gaps in patient experience and communicate them to the concerned departments.
- Collect patient/family feedback and coordinate appropriate action.
- Promote a respectful, compassionate, and patient-centered setting.

Pay: ₹20,503.75 - ₹30,168.15 per month
