Job Purpose

The Virtual Registered Nurse (RN) is responsible for conducting post-visit outreach to patients who have been seen by a provider, with the objective of clarifying visit outcomes, reinforcing care plans, addressing patient questions, and identifying any new or unresolved clinical or scheduling needs. This role supports a high-volume family medicine practice serving over 50,000 patients annually and plays a critical role in continuity of care, patient engagement, and care coordination,

Principal Accountabilities:

- Conduct outbound virtual calls to patients following in-office visits to review and clarify visit details, treatment plans, medications, and next steps.
- Review provider documentation and clinical notes within the EMR prior to patient interaction.
- Accurately document patient interactions, updates, and clarifications directly into the EMR.
- Identify and escalate new clinical concerns, follow-up requirements, or scheduling needs to the appropriate provider or care team.
- Manage a high daily caseload (80+ cases per day)



while maintaining quality, compliance, and patient satisfaction standards.
- Communicate promptly with internal teams in the event of system, connectivity, or access issues.
- Adhere to all HIPAA, data security, and client-specific compliance requirements.

Related experience/ must- have skills / qualifications:

- Active US Registered Nurse (RN) license – mandatory.
- Solid verbal communication and patient engagement skills.
- English language proficiency: minimum 8/10 for speaking and comprehension.
- Experience in Family Medicine or ambulatory care settings preferred.
- Ability to work independently in a remote/virtual care environment.
- Proven ability to manage high-volume clinical workflows efficiently Specific Tools / Platforms
- MEDENT EMR (secure remote access required).
- Avaya VoIP (CNY) and Ooma VoIP via CinchMD for patient calls.
- WhatsApp for internal coordination and communication.
- Secure standalone CPU/Laptop with reliable high-speed internet. Capabilities
- High attention to clinical accuracy and documentation quality.
- Strong time management and productivity in a monitored environment.
- Comfort with performance tracking and daily system monitoring.
- Ability to adapt to structured training programs and client-led workflows.
- Professionalism in patient interactions and cross-functional collaboration.
