Job Description

Job Summary

The Clinical Pharmacist is responsible for providing specialized pharmaceutical care to cancer patients by ensuring the safe, effective, and evidence-based use of chemotherapy, immunotherapy, targeted therapies, and supportive care medications. The role involves medication review, dose verification, chemotherapy preparation oversight, patient counseling, adverse event monitoring, and collaboration with oncologists and multidisciplinary healthcare teams to optimize patient outcomes.

Key Responsibilities

Clinical Responsibilities

- Review and validate oncology prescriptions for appropriateness, dosing accuracy, drug interactions, contraindications, and treatment protocols.
- Verify chemotherapy, immunotherapy, targeted therapy, and supportive care medication orders.
- Participate in multidisciplinary tumor boards and clinical rounds.
- Monitor patients for adverse drug reactions, toxicities, and treatment-related complications.
- Recommend dose modifications based on laboratory parameters, organ function, and patient-specific factors.
- Ensure compliance with established oncology treatment guidelines and protocols.
- Provide pharmacotherapeutic recommendations to physicians and healthcare professionals.

Medication Management

- Supervise and ensure protected preparation, handling, storage, and dispensing of cytotoxic medications.
- Maintain compliance with chemotherapy safety standards and hazardous drug handling guidelines.
- Review medication reconciliation during admission, transfer, and discharge.




- Monitor inventory and utilization of high-cost oncology medications.
- Assist in formulary management and medication procurement activities.

Patient Counseling & Education

- Counsel patients and caregivers regarding chemotherapy regimens, medication administration, side effects, precautions, and adherence.
- Educate patients on supportive care medications and symptom management.
- Provide written and verbal medication information and follow-up guidance.

Quality & Safety

- Participate in medication safety initiatives and quality improvement programs.
- Report and document medication errors, adverse drug reactions, and near-miss incidents.
- Conduct audits related to oncology pharmacy practices and chemotherapy administration.
- Ensure compliance with NABH, JCI, and other regulatory standards.

Documentation & Research

- Maintain accurate clinical documentation and pharmaceutical interventions.
- Participate in clinical research, drug utilization reviews, and pharmacovigilance activities.
- Assist in developing and updating oncology treatment protocols and policies.
- Contribute to continuing education and training programs for healthcare staff.

Qualifications

- Pharm D
- Registered Pharmacist with the State Pharmacy Council.
- Certification or specialized training in Oncology Pharmacy preferred.
