CARELY is hiring for post-discharge/ OPD consultation patient care services. The role involves patient onboarding in the Carely application portal, scheduling condition-based templates, daily follow-up calls, alert handling, clinical coordination & escalation, and subscription renewal counselling.
Key Responsibilities
Onboard patients using discharge summaries
Handle patient alerts and follow-ups
Make scheduled daily calls to patients/attendants
Coordinate with hospital doctors for clinical or emergency escalation
Counsel patients for subscription renewals
Maintain call logs and reports using MS Excel and other CRM based applications
Eligibility
Qualification: ANM / GNM / ANY
Experience: 1–2 years in hospital / tele-nursing / patient counselling preferred
Skills: MS Excel (Mandatory); calling CRM based apps knowledge preferred
Languages: , English & Hindi, Marathi
