Job Summary
We are seeking an experienced E&M; IP/OP Medical Coder responsible for reviewing clinical documentation and assigning accurate Evaluation & Management (E&M;) codes for Inpatient (IP) and Outpatient (OP) encounters. The role requires strong knowledge of medical coding guidelines, payer regulations, documentation standards, and quality assurance processes to ensure coding accuracy and compliance.
Key Responsibilities
- Review inpatient and outpatient medical records to assign appropriate E&M; codes based on documentation.
- Analyze physician documentation to ensure compliance with coding guidelines and regulatory requirements.
- Apply ICD-10-CM, CPT, HCPCS, and E&M; coding principles accurately.
- Identify documentation deficiencies and communicate queries to providers when required.
- Ensure coding quality, productivity, and turnaround time targets are met.
- Stay updated with changes in coding regulations, CMS guidelines, and payer-specific requirements.
- Participate in internal audits and quality improvement initiatives.
- Maintain confidentiality and comply with HIPAA and organizational policies.
- Support denial management by reviewing coding-related rejections and providing corrective actions.
Required Qualifications
- Graduate in Life Sciences, Nursing, Pharmacy, Medical Sciences,
or related healthcare field.
- Certification preferred: CPC, CCS, CCS-P, CRC, COC, or equivalent.
- Robust understanding of E&M; coding guidelines for inpatient and outpatient services.
- Knowledge of ICD-10-CM, CPT, HCPCS, and medical terminology.
- Excellent analytical, communication, and documentation review skills.
- Familiarity with EMR/EHR systems.
Experience
- 1-5+ years of experience in E&M; coding (Inpatient/Outpatient).
- Experience in US Healthcare Revenue Cycle Management (RCM) preferred.
- Audit and quality review experience is an added advantage.
