Job Title: MRD In-Charge (Medical Records Department)

Key Responsibilities
- Manage the day-to-day operations of the Medical Records Department.
- Ensure proper collection, verification, filing, indexing, storage, and retrieval of patient medical records.
- Maintain accurate and complete physical and electronic medical records.
- Ensure medical records are available to authorized departments and clinicians when required.
- Monitor discharge records and ensure timely completion and submission of patient files.
- Coordinate with doctors, nursing, billing, front office, insurance/TPA, and other departments for medical documentation.
- Ensure proper documentation of patient admissions, discharges, transfers, investigations, procedures, and treatment records.




- Maintain confidentiality and security of patient medical information in accordance with hospital policies and applicable regulations.
- Establish and maintain proper record-retention, tracking, and disposal procedures.
- Monitor incomplete, missing, or incorrectly documented patient files and coordinate with concerned departments for correction.
- Prepare and maintain MRD reports, statistics, registers, and management information as required.
- Support hospital audits, inspections, accreditation requirements, and quality-management activities.
- Ensure proper coding, indexing, and classification of medical records as applicable.
- Maintain records related to medico-legal cases and ensure they are handled according to hospital policy and legal requirements.
- Monitor the use, security, and availability of MRD files, registers, stationery, and equipment.
