Job Summary
The Clinical Pharmacist is responsible for providing secure, effective, and rational medication management by working closely with physicians, nurses, and other healthcare professionals. The role includes reviewing medication orders, monitoring therapy, identifying drug-related problems, providing medication information, and promoting patient safety.
Key Responsibilities
- Review and verify medication orders for appropriate drug, dose, route, frequency, and duration.
- Identify potential drug-drug interactions, drug-disease interactions, allergies, contraindications, and medication errors.
- Monitor patients for therapeutic effectiveness and adverse drug reactions (ADRs).
- Participate in clinical rounds and provide appropriate medication-related recommendations to the healthcare team.
- Provide medication counselling to patients and caregivers regarding dosage, administration, precautions, and possible side effects.
- Perform medication reconciliation during admission, transfer, and discharge.
- Monitor high-alert medications, antibiotics, anticoagulants, and other high-risk medicines as applicable.
- Support Antimicrobial Stewardship Program (ASP) activities and promote rational antibiotic use.
- Report, document,
and follow up on adverse drug reactions and medication errors.
- Provide accurate and evidence-based drug information to doctors, nurses, and other healthcare professionals.
- Participate in formulary management and evaluation of medication utilization.
- Assist in maintaining and updating medication-related policies, protocols, guidelines, and formularies.
- Monitor proper storage, handling, and usage of medications in clinical areas.
- Participate in Pharmacy & Therapeutics Committee activities and other relevant hospital committees.
- Maintain appropriate clinical pharmacy documentation and records.
- Conduct or assist with medication-use audits and quality improvement activities.
- Participate in patient safety, infection control, and hospital quality initiatives.
- Support staff education and training on medication safety and rational drug use.
- Maintain confidentiality of patient information and comply with hospital policies and applicable regulations.
- Perform other duties assigned by the Pharmacy In-charge / Management.
Qualifications
- Pharm.D from a recognized institution.
Pay: ₹15,000.00 - ₹29,875.27 per month
