Charge Entry Specialist Medical Documentation Job Description Who you are/: Precisely input patient demographic data, insurance information, and diagnostic/procedural codes into medical billing systems from encounter forms, superbills, or EHR interfaces. To avoid billing errors, cross/-reference recorded charges with provider guidelines, medical price schedules, National Correct Coding Initiative revisions, and insurance billing standards. Before sending out the final claim, review and fix front/-end claim denials and clearinghouse scrub problems. Keep an eye on charge entry queues every day to make sure that all clinical encounters are completed within payer timely filing restrictions and client/-defined charge lag windows.



To ensure that all patient interactions are billed with no uncaptured charges, reconcile daily entered charge totals against provider encounter logs and appointment schedules.
