Job Title: Patient Care Coordinator

:

The Patient Care Coordinator is responsible for managing patient interactions and ensuring a smooth and efficient experience throughout the healthcare process. This role involves working closely with patients, families, and healthcare providers to facilitate communication, coordinate treatment plans, and support patients in navigating their healthcare journey. The Patient Care Coordinator will assist in scheduling appointments, managing medical records, and addressing patient inquiries.

Key Responsibilities:

- Serve as the primary point of contact for patients, addressing their questions and concerns regarding their care.
- Coordinate appointment scheduling and follow-up care to ensure timely access to services.
- Collaborate with healthcare providers to develop and implement patient care plans.
- Maintain accurate patient records and documentation in compliance with healthcare regulations.
- Educate patients about treatment options, procedures, and potential outcomes to promote informed decision-making.
- Assist in insurance verification and pre-authorization processes and provide support to patients regarding coverage questions.
- Follow up with patients post-visit to ensure satisfaction and adherence to treatment plans.




- Facilitate communication between patients and medical staff, promoting a patient-centered approach to care.

Skills and Tools Required:

- Strong interpersonal and communication skills to effectively interact with patients, families, and healthcare providers.
- Excellent organizational skills to manage multiple tasks and prioritize responsibilities effectively.
- Proficiency in using electronic health record (EHR) systems and medical scheduling software.
- Basic knowledge of medical terminology and healthcare practices.
- Ability to work in a fast-paced workplace and handle sensitive information with confidentiality.
- Problem-solving skills to address patient concerns and ensure a positive experience.
- Empathy and compassion to support patients through their healthcare challenges.
- Familiarity with insurance processes and patient advocacy.

Education and Experience:

- A high school diploma is required; an associate?s degree or Bachelor?s degree in healthcare administration, nursing, or a related field is preferred.
- Previous experience in healthcare or patient services is an advantage.

This role plays a critical part in enhancing patient satisfaction and ensuring the delivery of high-quality healthcare services.
