Role Overview

Circle Health is looking for compassionate and clinically strong Registered Nurses with NCLEX certification to join our growing remote care management team.

This role involves supporting patients through remote monitoring, clinical coordination, preventive care guidance, and lifestyle support to improve overall health outcomes and reduce avoidable hospitalizations.

The selected candidate will work closely with healthcare providers, care coordinators, and AI-driven healthcare tools to deliver personalized and high-quality patient care in a remote environment.

Key Responsibilities

- Provide remote patient care and clinical support
- Conduct patient follow-ups and health assessments
- Monitor patient conditions and identify potential health risks
- Support chronic care management and preventive care initiatives
- Coordinate empathetically with patients and healthcare providers
- Assist in reducing avoidable hospitalizations through proactive monitoring
- Document patient interactions and clinical observations accurately
- Use AI-driven healthcare tools and platforms for care management
- Support patients with lifestyle coaching and adherence to care plans




- Collaborate with cross-functional healthcare teams for better patient outcomes

Required Qualifications

- NCLEX certification is mandatory
- Active nursing license
- Experience in patient care and clinical coordination
- Strong understanding of preventive care and chronic care management
- Ability to conduct risk assessments and patient coaching
- Comfortable working in a remote setup
- Strong communication and interpersonal skills
- Ability to work independently and manage patients remotely

Preferred Qualifications

- Experience in long-term care, telehealth, or chronic care management
- Familiarity with EMR/EHR platforms and remote healthcare tools
- Bachelor’s degree in Nursing or related healthcare field preferred
- Exposure to AI-driven healthcare platforms is an added advantage
