1. 24-Hour Post-Discharge Follow-Up

Complete first follow-up call within 24 hours of discharge.

Document every call in CRM/SharePoint tracker.

Assess medication adherence, behavioural stability, family comfort, and immediate concerns.

Missed follow-up or documentation = major lapse.

2. OPD Continuity & Appointment Booking

Ensure timely OPD appointment booking after discharge.

Coordinate with Psychiatrist and Clinical Psychologist.

Send 24-hour appointment reminders.

Track OPD attendance and follow-up compliance.

Address missed/non-booked appointments proactively.

3. Long-Term Follow-Up

Follow-up at 24 hours, 7 days, 30 days, and 90 days.

Make at least 3 documented attempts before marking unreachable.

Maintain updated tracker and flag relapse signs immediately.

4. Feedback & Google Reviews

Collect feedback from all discharged patients/families.

Request Google reviews from satisfied families.

Maintain monthly review target.





Escalate negative feedback within 24 hours.

5. Readmission & Reactivation

Identify high-risk/relapse patients.

Recommend reassessment when required.

Follow up for readmission/reactivation opportunities.

Escalate relapse concerns immediately.

6. Reporting & Escalation

Submit weekly continuity tracker.

Report OPD conversion, attendance, and readmission funnel.

Escalate high-risk cases, medication non-compliance, and family distress.

7. Attendance & Professional Discipline

Maintain required attendance as per company policy.

Follow shift timings and leave procedures.

Ensure proper handover before leave.

Immediately report emergency leave.

Prior approval required for absence on critical operational days.

Pay: ₹15,000.00 - ₹20,000.00 per month
