Job Summary:
We are looking for a qualified and detail-oriented Clinical Pharmacist to support patients and healthcare teams by ensuring safe and effective medication use. The candidate will be responsible for reviewing prescriptions, providing medication guidance, monitoring drug therapy, and maintaining accurate medication records.
Key Responsibilities:
Review and validate prescriptions for accuracy and appropriateness.
Provide patients with transparent information regarding medications, dosage, timing, and precautions.
Monitor medication therapy and identify potential drug interactions or adverse effects.
Coordinate with doctors and healthcare professionals regarding medication-related concerns.
Maintain accurate patient medication records and documentation.
Ensure compliance with pharmacy and healthcare regulations.
Counsel patients on proper medication usage and adherence.
Support medication reconciliation and follow-up activities.
Maintain knowledge of new medicines, therapies, and clinical guidelines.
Eligibility & Skills:




B.Pharm / M.Pharm / Pharm.D from a recognized institution.
Valid pharmacy registration/license as applicable.
Clinical pharmacy or hospital pharmacy experience preferred.
Good knowledge of medicines, drug interactions, and dosage calculations.
Strong communication and patient-counselling skills.
Good documentation and computer skills.
Ability to work effectively with doctors, nurses, and other healthcare professionals.
