Role Overview

We are looking for Health Coach/ Care Manager who can support execution of personalized care plans through structured follow-ups, customer engagement, and care coordination. This role is suited for individuals who are comfortable interacting with customers, can take ownership of tasks under guidance, and are keen to build a career in healthcare delivery and care management, while working closely with clinical and customer success teams. You will support the team in ensuring customers stay on track with their care plans and receive a seamless experience.

Key Responsibilities

1. Care Plan Execution Support
- Assist in translating care plans into straightforward, actionable steps for customers.
- Track care plan progress and ensure timely completion of assigned tasks.
- Maintain structured trackers for pending tasks and next actions.

2. Customer Engagement Support
- Engage with customers via calls and WhatsApp, for follow-ups and reminders.




- Ensure customers clearly understand next steps and required actions.
- Build comfort and trust through consistent and empathetic interactions.

3. Care Coordination Support
- Coordinate with internal teams (clinics, diagnostics, specialists) for appointments and follow-ups.
- Assist in identifying missed actions, delays, or potential health risks/concerns, and ensure timely escalation to the relevant team.
- Support smooth execution of care plans across touchpoints.

4. Communication & Documentation
- Maintain clear and structured communication with customers and internal teams.
- Ensure proper documentation of interactions and updates in CRM.
- Support reporting and tracking of customer progress.

Pay: ₹350,000.00 - ₹450,000.00 per year
