A. Programme Planning & Implementation
- Develop annual, quarterly, and monthly programme plans in alignment with Sankalp’s organisational objectives.
- Translate programme goals into measurable activities, deliverables, timelines, and responsibilities.
- Ensure smooth implementation of the Thalassemia Management Programme across all designated centres.
- Monitor programme activities and ensure adherence to approved processes, protocols, and SOPs.
- Identify operational gaps and develop appropriate corrective and preventive actions.
- Ensure continuity of patient services and minimise disruptions to critical care activities.
- Review programme requirements and recommend improvements in processes, staffing, infrastructure, and resources.
B. Patient-Care Programme Coordination
- Ensure that patients enrolled in the programme receive timely and coordinated support.
- Monitor key patient-care activities such as:
- Regular blood transfusions
- Chelation adherence and monitoring
- Laboratory investigations
- Complication screening
- Patient counselling
- Follow-up and appointment adherence
- Referral and coordination with treating hospitals/doctors
- Ensure proper documentation and tracking of patient-related activities.
- Monitor missed appointments, delayed investigations, and other patient-care gaps and ensure timely follow-up.
- Coordinate with medical and nursing teams to address operational issues affecting patient care.
- Ensure that patient concerns and grievances are appropriately addressed and escalated when required.
C. Team Management & Leadership
- Lead, supervise, and support the programme team, including coordinators, counsellors, nurses, social workers, administrative staff, and other programme personnel.
- Define responsibilities and ensure clarity of roles within the team.
- Conduct regular team meetings and review programme performance.
- Set individual and team objectives in consultation with reporting managers.
- Monitor employee performance, attendance, productivity, and work discipline.
- Identify training and capacity-building requirements.
- Mentor team members and encourage professional development.
- Address performance or behavioural concerns in coordination with HR and senior management.
- Ensure adequate manpower planning and support recruitment requirements.
D. Monitoring, Evaluation & Performance Management
- Develop and monitor programme Key Performance Indicators (KPIs).
- Track patient enrolment, retention, transfusion adherence, follow-up, investigations, counselling, and other programme indicators.
- Prepare regular programme dashboards and performance reports.
- Analyse trends, identify gaps, and recommend corrective actions.
- Conduct periodic programme reviews with the team.
- Establish systems for tracking action points and ensuring timely closure.
- Evaluate programme effectiveness and recommend evidence-based improvements.
E. Data Management & Documentation
- Ensure accurate, complete, and timely entry of programme and patient data.
- Maintain appropriate programme records and documentation.
- Ensure confidentiality and responsible handling of patient information.
- Coordinate with MIS/data teams to ensure accuracy of reports.
- Conduct periodic data verification and identify discrepancies.
- Develop and maintain appropriate trackers and dashboards for programme monitoring.
- Ensure documentation is audit-ready and readily available when required.
F. Financial & Resource Management
- Prepare programme budgets in coordination with the Finance and Senior Management teams.
- Monitor programme expenditure against approved budgets.
- Ensure optimum utilisation of programme resources.
- Review financial requirements and provide timely inputs for budget planning.
- Monitor programme-related procurement and resource requirements.
- Ensure compliance with financial policies and approval processes.
- Identify opportunities for cost optimisation without compromising patient care or programme quality.
G. Donor & Stakeholder Management
- Coordinate with donors, funding partners, hospitals, laboratories, blood centres, government agencies,
and other stakeholders as required.
- Prepare timely programme updates, reports, presentations, and impact reports for donors and management.
- Ensure donor commitments and reporting requirements are met within agreed timelines.
- Provide accurate programme data and success stories for donor communication.
- Maintain professional and effective relationships with key stakeholders.
- Represent Sankalp in relevant meetings, workshops, conferences, and programme reviews when delegated.
H. Quality, Compliance & Risk Management
- Ensure programme activities comply with organisational policies, SOPs, applicable statutory requirements, and quality standards.
- Work closely with the Quality team to identify and address process gaps.
- Conduct periodic internal programme reviews.
- Identify operational, patient-care, data, financial, and compliance risks.
- Develop mitigation plans and monitor implementation.
- Ensure timely closure of audit observations and corrective actions.
- Promote a culture of quality, accountability, patient safety, and continuous improvement.
I. Coordination with Medical & Clinical Teams
- Coordinate with treating physicians, consultants, nurses, counsellors, laboratories, blood centres, and hospital teams.
- Facilitate smooth communication between programme and clinical teams.
- Escalate patient-care or clinical concerns to the appropriate medical authority.
- Ensure that the Project Manager does not independently make clinical decisions outside their professional competence.
- Support implementation of approved clinical and programme protocols.
J. Process Improvement
- Regularly review existing programme processes and identify opportunities for improvement.
- Develop and implement SOPs, workflows, checklists, and monitoring systems where required.
- Use programme data to identify recurring problems and develop sustainable solutions.
- Promote technology-based systems for patient tracking, reporting, communication, and monitoring.
Pay: ₹35,000.00 - ₹60,000.00 per month
