About the Role

We are seeking a detail-oriented and experienced QA Documentation Specialist to join our Home Health team. This position is responsible for reviewing clinical and administrative documentation to ensure accuracy, completeness, consistency, and compliance with applicable home health regulations and agency policies.

The ideal candidate will have strong knowledge of Home Health documentation, OASIS, Medicare requirements, and clinical record review, with excellent attention to detail and the ability to identify documentation gaps and compliance risks.

Key Responsibilities

* Review patient charts and clinical documentation for accuracy, completeness, and compliance.

* Perform quality assurance audits of home health records, including skilled nursing, therapy, and other clinical documentation.

* Review OASIS documentation and plans of care for consistency and completeness.

* Identify missing, incomplete, or inconsistent documentation and communicate findings to clinical staff.

* Monitor documentation for compliance with Medicare, CMS, state regulations, and agency policies.

* Ensure documentation supports medical necessity, skilled services, and the plan of care.

* Track QA findings and identify recurring documentation issues or trends.

* Provide feedback and education to clinicians to improve documentation quality.

* Assist with preparation for internal audits, external surveys,



and regulatory reviews.

* Maintain accurate QA records and prepare reports for management.

* Collaborate with clinical and administrative teams to improve overall documentation processes.

Qualifications

* Previous experience in Home Health QA, clinical documentation review, or compliance preferred.

* Knowledge of Medicare Home Health regulations and CMS requirements.

* Familiarity with OASIS and Home Health Conditions of Participation.

* Strong understanding of clinical documentation standards.

* Excellent attention to detail and organizational skills.

* Ability to work independently and meet deadlines.

* Strong written and verbal communication skills.

* Experience with Home Health EMR systems preferred.

Preferred Experience

* coding / Billing /Medical professionals , or other healthcare professional with Home Health experience preferred.

* Experience with OASIS review and Home Health chart audits.

* Experience preparing documentation for Medicare or regulatory audits.

* Knowledge of Home Health billing and documentation requirements is a plus.
