Role & responsibilities
Coordinate all pre-transplant, transplant, and post-transplant activities for heart and lung transplant patients.
Act as the primary point of contact for transplant patients, families, donors, and multidisciplinary teams.
Coordinate patient evaluations, investigations, consultations, admissions, and transplant scheduling.
Maintain transplant waiting lists and ensure compliance with transplant registry and allocation guidelines.
Coordinate with transplant surgeons, physicians, intensivists, nursing teams, laboratories, blood banks, and diagnostic departments.
Ensure timely completion of transplant documentation, consent forms, insurance approvals, and regulatory requirements.
Educate patients and families regarding transplant procedures, medications, follow-up care, lifestyle modifications, and post-transplant precautions.
Monitor patient follow-up schedules and coordinate post-transplant care and continuity of treatment.
Maintain accurate patient records, transplant databases, and clinical documentation.




Coordinate organ retrieval logistics and communication with organ sharing networks and external agencies when required.
Ensure adherence to hospital policies, transplant protocols, ethical standards, and government regulations.
Support quality initiatives, audits, accreditation standards, and transplant program evaluations.
Assist in data collection, reporting, clinical research activities, and transplant outcome analysis.
Coordinate transplant-related meetings, CMEs, patient counseling sessions, and awareness programs.
Ensure effective communication and coordination during emergency transplant procedures.
Monitor and report transplant program performance indicators and operational metrics.
Maintain confidentiality of patient information and sensitive medical records.
Perform additional responsibilities assigned by the transplant team or hospital leadership.
