Key Responsibilities
Patient Assessment & Care
Review patient medical records and medication histories to determine appropriateness of therapy.
Evaluate patient progress and adjust drug dosages or frequencies based on lab results and clinical response.
Educate patients on their medications, including proper administration routes, potential side effects, and adherence tips. [1, 2, 3, 4, 5, 6]
Collaboration with Healthcare Teams [1]
Participate in ward rounds, medical team meetings, and interdisciplinary care planning.
Recommend the safest, most cost-effective, and evidence-based medications for individual patient conditions (such as diabetes, hypertension, or infectious diseases).
Assist in developing hospital-wide clinical pharmacy programs and medication policies. [1, 2, 3, 4, 5]
Medication Safety & Compliance
Prevent medication errors, adverse drug events, and drug interactions.
Document medication plans and clinical interventions thoroughly in patient electronic health records.
Provide expert drug information and pharmacotherapy guidance to the medical and nursing staff. [1, 2, 3, 4]
Qualifications & Requirements
Education: A Doctor of Pharmacy (Pharm.D.) degree or relevant advanced degree in pharmacology.
Licensing: Valid state or national licensure to practice as a pharmacist.
Experience: Hands-on clinical experience, often acquired through a post-graduate pharmacy residency program (such as PGY-1 or PGY-2) or specialized hospital practice.
Key Skills: High-level critical thinking, solid evidence-based medicine knowledge, and exceptional communication skills.
Pay: ₹23,000.00 - ₹35,000.00 per month
