Hospital Pharmacist

1. Department

Pharmacy / Clinical Services

1. Reports To

Pharmacy Manager / Medical Superintendent / Hospital Administrator

1. Key ResponsibilitiesA. Prescription &
- Dispensing

- Review prescriptions for completeness, dosage, drug interactions, contraindications, and allergies.

- Dispense medicines accurately as prescribed by authorized medical practitioners.

- Provide patients/caregivers with appropriate instructions regarding dosage, administration, storage, and precautions.

- Maintain confidentiality of patient and prescription information.

- Clarify prescription-related issues with doctors whenever required.

B. Inpatient &

- Outpatient Pharmacy

- Manage dispensing of medicines to OPD and IPD patients.

- Supply medicines to wards, ICU, OT, emergency, and other hospital departments as per authorized requisitions.

- Ensure correct medicine, strength, dosage form, quantity, and expiry before dispensing.

- Maintain proper documentation of all medicine issues and returns.

C. Inventory &

- Stock Management

- Monitor medicine stock levels and prepare purchase/requisition requirements.

- Maintain minimum and maximum stock levels for critical and essential medicines.

- Follow FEFO (First Expiry, First Out) principles.

- Regularly check expiry dates, damaged stock, and slow/non-moving medicines.

- Maintain accurate physical and system-based inventory records.

- Participate in periodic physical stock verification.

D. Controlled / High-Risk Medicines

- Maintain prescribed records for narcotic, psychotropic, Schedule H/H1,



and other regulated medicines as applicable.

- Ensure secure storage and authorized issue of controlled/high-risk medicines.

- Report discrepancies, losses, or unauthorized access immediately to the appropriate authority.

E. Quality &

- Patient Safety

- Ensure medicines are stored under prescribed temperature, humidity, light, and other conditions.

- Monitor refrigerator/cold-chain temperatures where applicable.

- Identify and report medication errors, adverse drug reactions, and suspected counterfeit/substandard medicines.

- Participate in medication-safety and quality-improvement initiatives.

- Follow hospital infection-control and patient-safety policies.

F. Documentation &

- Compliance

- Maintain pharmacy registers, stock records, purchase records, issue records, expiry records, and other statutory documentation.

- Ensure compliance with applicable provisions of the Drugs and Cosmetics Act/Rules, Pharmacy Act, NDPS requirements, and other applicable regulations.

- Maintain records required for hospital accreditation and internal/external audits.

- Assist during inspections by regulatory and accreditation authorities.

G. Coordination

- Coordinate with doctors, nurses, procurement, stores, accounts,



and hospital administration.

- Communicate medicine availability and shortages to concerned departments.

- Assist in developing and updating the hospital formulary and approved medicine list.

- Participate in Pharmacy &
- Therapeutics Committee activities, where applicable.

1. Daily Operational Duties

- Check opening stock and critical medicine availability.

- Review prescriptions/requisitions and dispense medicines.

- Verify medicine batches, expiry dates, and storage conditions.

- Update ERP/pharmacy software and relevant registers.

- Monitor near-expiry and slow-moving stock.

- Maintain cleanliness, organization, and security of the pharmacy.

- Prepare shift handover and report any significant stock or medication issues.

1. Qualifications

- B.Pharm / D.Pharm from a recognized institution.

- Valid registration with the State Pharmacy Council.

- Experience in a hospital pharmacy is preferred.

- Knowledge of hospital pharmacy operations and applicable drug regulations.

1. Required Skills

- At least 2-3 years experience in any reputed hospital

- Strong knowledge of medicines and dosage forms.

- Prescription-reading and dispensing accuracy.

- Inventory and stock-management skills.

- Positive computer/ERP knowledge.

- Strong attention to detail.

- Patient-focused communication.

- Ability to work under pressure and in shifts.

- Good coordination and documentation skills.

Pay: ₹15,000.00 - ₹25,000.00 per month
