Position Title: Pharmacist

Reporting To: Unit Manager
Supervises: Pharmacy Assistant / Store Executive

Role Purpose

To ensure secure, accurate, and continuous medication management for residents. The Pharmacist is responsible for medication dispensing, therapy monitoring, compliance, inventory control, and Nurses/Caretaker education, supporting high-quality, dignified, and compliant care across hospital and residential healthcare settings.

Key Responsibilities
Medication Management & Dispensing
Dispense medications accurately for residents as per physician prescriptions.
Review medication charts for dosage accuracy, drug interactions, duplications, and contraindications.
Support unit-dose dispensing, medication carts, and resident-specific medication packing where applicable.
Ensure timely medication availability for routine, PRN, and emergency requirements.
Clinical Pharmacy & Therapy Monitoring
Monitor ongoing drug therapies, especially for chronic conditions common in geriatric and long-term care populations.




Identify and escalate adverse drug reactions, side effects, and medication errors.
Provide medication-related inputs to doctors, nurses, and care teams during rounds or care reviews.
Support deprescribing and medication optimization initiatives for senior residents.
Resident, Family & Caregiver Education
Counsel residents, family and Nurses/caretaker on medication usage, dosage schedules, and precautions.
Educate nursing staff on medication administration protocols, storage, and safety.
Promote medication adherence and awareness in assisted living and LTC settings.
Pharmacy Operations & Inventory Control
Maintain accurate inventory of medications, including controlled drugs and emergency stocks.
Track expiry, batch numbers, and recalls; ensure removal of expired or damaged medicines.
Coordinate with procurement for timely replenishment and stock optimization.
