About the Role

We’re looking for a US Healthcare Interoperability Engineer: FHIR/HL7 with hands-on experience in FHIR, HL7 v2, healthcare systems, and software development.

The role combines healthcare domain knowledge with practical engineering. You’ll work alongside our existing development team on custom healthcare software and EHR/EMR platforms, contributing to healthcare workflows, interoperability, integrations, and production code.

This is a hands-on engineering role. Candidates whose experience is limited to healthcare analysis, implementation, configuration, documentation, or vendor coordination without software development experience are not a fit.

Responsibilities

- Design and develop healthcare interoperability solutions using FHIR and HL7 v2.
- Build and maintain production healthcare integrations and APIs.
- Write production code for healthcare data exchange, transformation, validation, and messaging.
- Work with EHRs, laboratories, pharmacies, clearinghouses, and other healthcare systems.
- Understand clinical and administrative workflows and translate requirements into technical implementations.
- Design FHIR-based data models, APIs, and integration architectures.
- Define and implement data mapping and transformation logic.
- Develop integration workflows with appropriate error handling, retries, reconciliation, logging, and monitoring.
- Work closely with developers on healthcare-specific backend and application functionality.
- Participate in healthcare data migration and interoperability projects.
- Troubleshoot complex healthcare integration and data-exchange issues.
- Review technical implementations for interoperability and healthcare data correctness.




- Contribute directly to application and backend development when required.

Required Qualifications

- 3+ years of professional software engineering experience.
- Strong hands-on experience with FHIR in production healthcare systems.
- Strong hands-on experience with HL7 v2 in production healthcare systems.
- Proven experience personally writing and maintaining production code for healthcare integrations, interoperability, APIs, or healthcare applications.
- Practical understanding of EHR/EMR systems and clinical workflows.
- Robust understanding of REST APIs and backend systems.
- Experience with healthcare data mapping, transformation, validation, and reconciliation.

Software Engineering + FHIR/HL7 + US Healthcare Domain Knowledge

You should be able to take a real-world healthcare requirement and determine how it should be implemented technically.

For example

“When a provider orders a laboratory test during an encounter, the order should be transmitted electronically, the result should automatically return to the patient’s chart, abnormal results should be appropriately flagged, and a follow-up task should be generated.”

You should be able to:

- Understand the underlying workflow
- Identify the appropriate FHIR resources and/or HL7 messages
- Design the integration and data flow
- Define mappings and transformations
- Determine the required APIs and backend components
- Identify clinical and technical edge cases
- Work with the development team on implementation
- Contribute directly to the production code

This Is a Hands-On Engineering Role

We’re not looking for candidates whose experience is primarily limited to:

- Healthcare business analysis
- EHR implementation or configuration
- Interface project management
- Vendor coordination
- FHIR/HL7 documentation or mapping without coding
- Non-technical healthcare consulting

Candidates must have demonstrated hands-on software development experience.

You do not need to specialize in a specific programming language. Experience with Node.js/TypeScript, Java, C#, Python, or another modern development stack is acceptable.
