Job Responsibilities

- Prepare and type accurate discharge summaries as per the prescribed hospital format.

- Type discharge summaries based on doctors’ handwritten notes, dictated content, or electronic records.

- Ensure correct patient demographics, diagnosis, procedures, investigations, treatment details, and discharge instructions.

- Coordinate with consultants/doctors for clarification and completion of discharge summaries.

- Maintain accuracy, confidentiality, and proper documentation of patient records.

- Ensure timely completion of discharge summaries to support smooth patient discharge.

- Review documents for spelling,



grammar, medical terminology, and formatting errors.

- Maintain proper records and follow hospital policies and documentation standards.

- Coordinate with the Medical Records Department, Nursing, Billing, and other concerned departments when required.

Qualification

- Graduate / Diploma in any relevant discipline.

- Knowledge of medical terminology will be preferred.
