Role & responsibilities A Home Health Medical Coder is responsible for reviewing clinical documentation and assigning accurate ICD-10-CM diagnosis codes for home health patients. The coder ensures compliance with CMS Home Health Prospective Payment System (HH PPS), OASIS documentation requirements, and payer-specific guidelines to support accurate reimbursement and quality reporting.

Job Title: Home Health Medical Coder

Key Responsibilities

- Review and analyze home health documentation, including:

- OASIS assessments
- Start of Care (SOC) records

- Recertifications

- Resumption of Care (ROC)

- Discharge summaries
- Physician orders and clinical notes
- Assign accurate ICD-10-CM diagnosis codes according to CMS and coding guidelines.
- Determine primary and secondary diagnoses based on documentation and coding conventions.




- Ensure coding accurately reflects the patient's medical condition, comorbidities, and treatment plan.
- Review OASIS assessments for diagnosis coding consistency and accuracy.
- Support Home Health Prospective Payment System (HH PPS) reimbursement requirements.
- Ensure compliance with CMS, Medicare, Medicaid, HIPAA, and payer regulations.
- Identify documentation deficiencies and communicate with clinicians for clarification when necessary.
- Participate in coding audits and quality assurance activities.
- Monitor coding updates and regulatory changes impacting home health services
