About the Role

Location : Remote Role (WFH)

- Horizon One: Fix Assessment. Adult ADHD and autism services face massive waiting lists, with clinicians spending a large portion of every assessment on structured documentation that AI can now automate properly. Our platform runs the full pathway: digital screening, collateral information collection, and an ambient-AI clinical assessment that produces a completed standardized diagnostic framework (e.g., DIVA-5) and draft psychiatric report for clinician review. The clinician stays in the loop and signs off on all documentation — compressing the admin, not the judgment.

- Horizon Two: The Mental Health OS. Assessment is the entry point, not the destination. Every assessment generates structured longitudinal data about a patient that typically gets flattened into unsearchable free text. We are building toward a clinical operating system where a patient's history is structured and instantly legible across diagnosis, treatment, risk, and follow-up rather than treated as an isolated episode.

You will be our second engineering hire and the owner of how this gets built. Early on, that means writing core production code: LLM pipelines with real evaluation discipline, a secure clinical data layer, and end-to-end assessment workflows. You will also make the architectural decisions that enable Horizon Two. Within 12 months, you will hire and lead a small engineering team and own our technical assurance and regulatory compliance posture.

This is a ground-floor role with founding-level equity, with a pathway to the CTO title based on defined milestones.

What You'll Do





- Own the architecture and ship production code across the entire platform.

- Build LLM pipelines for clinical summarization, structured extraction, and ambient transcription, incorporating rigorous evaluation harnesses rather than relying on unmonitored outputs.

- Design the longitudinal clinical data model that Horizon Two depends on.

- Build a secure, compliant data layer: locally hosted/region-compliant, encrypted, fully auditable, and based on least-privilege access.

- Drive security and clinical safety compliance work alongside our clinical safety team.

- Hire and lead the first 2–3 engineers over the next 12 months.

What We're Looking For

- Backend & Cloud Mastery: 6 years of deep, hands-on backend and cloud experience (Python, TypeScript, AWS, PostgreSQL) with a track record of shipping production code.

- Production GenAI / LLMs: Practical experience building LLM applications, including evaluation, structured data extraction, RAG, and failure-mode handling.

- Regulated Domain Experience: Experience building software in a regulated environment (healthcare, fintech, or similar) where audit trails, data protection, and evidence-backed compliance matter.

- Platform & Product Architecture: Ability to balance rapid delivery of user-facing features while designing scalable data models for long-term platform needs.

- Execution & Leadership: Comfort with ambiguity, direct clinical collaboration, and a preference for equity and long-term ownership.

Nice to Have

- Healthcare or medical device background (FHIR, HL7, HIPAA, CE marking, or equivalent standards).

- Speech-to-text or ambient documentation experience.

- Prior experience taking a product through security certifications (ISO 27001, SOC 2, or regional healthcare security frameworks) from scratch.
