Role & responsibilities

- Monitor and review physician medication orders received daily via eFax.
- Verify that each order is complete, legible, and clinically appropriate before it is acted on.
- Ensure new and updated medication orders are transmitted promptly and accurately to the dispensing pharmacy.
- Communicate medication changes, new orders, and discontinuations to caregiving staff in a transparent and timely manner.
- Review medication orders for dosage accuracy, drug interactions, duplications, and contraindications; flag and follow up on any order that doesn't make clinical sense with the prescribing physician's office.
- Reconcile medication administration records (MARs) against current physician orders.
- Respond to caregiver questions regarding medication administration, timing, and side effects.
- Maintain accurate, organized documentation of all medication orders and communications.
- Maintain resident confidentiality and HIPAA compliance in all communications





Preferred candidate profile The Remote Consultant Pharmacist is responsible for the daily review and processing of physician medication orders, coordinating with the dispensing pharmacy, and ensuring caregiving staff have accurate, up-to-date medication information for every resident. This role is a critical clinical safeguard in our medication management workflow, catching order errors, interactions, and inconsistencies before they reach residents.

Important Requirements

- Strong experience in medication management is required. Candidates should have hands-on experience reviewing medications, identifying potential medication-related issues, and supporting appropriate medication management.
- This is an hourly-based consulting position with variable payment, based on the hours/services required.
