This is a unique opportunity to be part of our first on-site office in India, located at the emerging IT hub in Tirupati, Andhra Pradesh (APEITA IT Park). While the initial 3–4 months will be remote, we are excited to build a collaborative hybrid workplace in Tirupati.

The Role

We are looking for a skilled Backend Engineer with strong experience in .NET, C#, SQL Server, and API development to build scalable backend systems for our AI-powered healthcare platforms. You will work on complex SQL queries, stored procedures, healthcare/EHR integrations, and modern AI applications. Experience with React.js is expected to enable effective collaboration with frontend engineers and end-to-end troubleshooting.

Key Responsibilities

- Backend Development: Design, develop, and optimize backend applications using C# and .NET, .NET 8. Build scalable REST APIs, implement business logic using clean architecture and design patterns, and develop reusable services, repositories, and middleware. Work with ASP.NET Core Web API, Entity Framework Core, and Dapper. Implement authentication and authorization using JWT, OAuth 2.0, and role-based access control.

- Database Development: Strong hands-on experience with Microsoft SQL Server. Design and optimize database queries, views, functions, and complex Stored Procedures (CTEs, temporary tables, TVPs, transactions, window functions, dynamic SQL, JSON/XML processing). Analyze and resolve SQL performance issues including deadlocks, blocking, and indexing problems. Perform database migrations and schema changes safely across environments.

- API Development & Integration: Develop and integrate REST APIs with internal and external systems. Experience with JSON/XML, OAuth 2.0, JWT, webhooks, third-party APIs, payment APIs, cloud APIs, and healthcare APIs. Capable of understanding third-party API documentation, creating integration services,



handling authentication/token management, and troubleshooting integration failures.

- React / Frontend Knowledge: Working knowledge of React.js fundamentals, TypeScript/JavaScript, components, hooks, state management, API integration (Axios/fetch), authentication flow, browser developer tools, network debugging, and CORS. Ability to understand how the React frontend communicates with the .NET backend and troubleshoot across the complete request/response flow.

- Cloud & DevOps (Mandatory): Candidates must have hands-on, mandatory experience with:

(1) deploying and managing applications on a cloud platform (Microsoft Azure preferred – App Services, Blob Storage, Azure SQL, Key Vault, Functions, Application Insights; AWS also acceptable) (2) creating and maintaining CI/CD pipelines (Azure DevOps, GitHub Actions, or similar) for automated build, test, and deployment

(3) Git version control including branching, merging, pull requests, and resolving merge conflicts (GitHub/Azure DevOps). Familiarity with Docker and IIS is also expected.

- AI Integration: Experience integrating AI capabilities (OpenAI or other LLM APIs) into backend applications. Responsibilities include building AI-powered backend services, implementing prompt-based workflows, working with structured AI/JSON responses, and implementing AI-assisted clinical/document processing, summarization, and classification workflows. Experience with embeddings, vector databases, and RAG (Retrieval-Augmented Generation) is a plus.

- Healthcare / EHR Integrations (Preferred): Experience with EHR/EMR systems, patient records, demographics, encounters, appointments, clinical notes, vitals, medications, and diagnoses. Knowledge of healthcare standards such as HL7, FHIR, SMART on FHIR, and OAuth 2.0 healthcare authentication is a strong advantage. Experience with platforms like Athenahealth, Epic, Cerner/Oracle Health, or similar is highly desirable.

- Security: Implement secure backend applications following industry best practices. Knowledge of authentication & authorization, JWT, OAuth 2.0, role-based access control, API security, encryption, secrets management, CORS, HTTPS,



input validation, SQL injection prevention, and OWASP principles. Knowledge of HIPAA/HITECH requirements is a strong advantage for healthcare projects.

Qualifications

- Bachelor's degree in Computer Science, Software Engineering, Information Technology, or related field
- 3+ years of professional backend development experience (max 5 years)
- Strong experience building production-grade .NET APIs
- Strong SQL Server and Stored Procedure experience
- Experience integrating multiple third-party APIs
- Hands-on experience deploying and managing applications on a cloud platform (Microsoft Azure or AWS)
- Hands-on experience creating and maintaining CI/CD pipelines (Azure DevOps, GitHub Actions, or similar)
- Hands-on experience with Git version control and collaborative workflows (branching, pull requests, merge conflict resolution.
- Experience with healthcare or medical applications (preferred)
- Experience with AI/LLM integrations (preferred)
- Valuable understanding of software architecture and design patterns
- Strong debugging and problem-solving skills

Ideal Candidate The ideal candidate is a backend-focused .NET engineer who can work across the complete technology stack—from React UI → REST API → .NET Business Logic → Dapper/EF Core → SQL Server/Stored Procedures → External API/EHR → AI Services. They should be comfortable investigating an issue from the frontend request all the way through the API, database, third-party integration, and external services. Healthcare/EHR integration experience and AI integration experience will be considered a major advantage.
