Job Responsibilities
Prepare and type accurate discharge summaries as per the prescribed hospital format.
Type discharge summaries based on doctors handwritten notes, dictated content, or electronic records.
Ensure correct patient demographics, diagnosis, procedures, investigations, treatment details, and discharge instructions.
Coordinate with consultants/doctors for clarification and completion of discharge summaries.
Maintain accuracy, confidentiality, and proper documentation of patient records.
Ensure timely completion of discharge summaries to support smooth patient discharge.
Review documents for spelling, grammar, medical terminology, and formatting errors.
Maintain proper records and follow hospital policies and documentation standards.




Coordinate with the Medical Records Department, Nursing, Billing, and other concerned departments when required.

Qualification
Graduate / Diploma in any relevant discipline.
Knowledge of medical terminology will be preferred.
