Position Summary: The Medical Coder assigns accurate diagnosis and procedure codes for emergency medicine and hospital medicine professional services based on provider documentation and applicable coding requirements.

- APC - Certification
- Experience in emergency

Required Experience: 3 to 5 years.

Essential Responsibilities

- Review medical records and provider documentation.
- Assign appropriate ICD-10-CM, CPT, and HCPCS codes.
- Apply applicable modifiers and place-of-service requirements.
- Code emergency medicine or hospital medicine encounters according to assigned scope.
- Identify incomplete, conflicting,



or insufficient documentation.
- Submit compliant provider queries through approved channels.
- Resolve coding edits and support coding-related denials.
- Follow official guidance, payer policies, company standards, and client requirements.
- Participate in audits, education, and corrective-action activities.
- Maintain required credentials and continuing education.

Required Qualifications

- Current CPC, CCS, CCS-P, COC, or an equivalent approved credential
- At least two years of skilled coding experience
- Experience in both emergency medicine and hospital medicine
- Strong knowledge of ICD-10-CM, CPT, HCPCS, modifiers, and coding guidelines
- Ability to interpret provider documentation accurately
- Strong written English and documentation skills

Preferred Qualifications

- Experience with coding audits and appeals
- Familiarity with payer edits and medical-necessity issues
- Experience working in an offshore delivery environment
