Role Description The Case Manager is responsible for coordinating and overseeing individualized care plans for clients, ensuring seamless support across clinical, psychological, and social services. This full-time, on-site role in the Greater Coimbatore Area involves conducting intake assessments, gathering client histories, and collaborating with psychiatrists, psychologists, therapists, and other team members to develop and monitor treatment goals.

The Case

Manager will maintain accurate and timely documentation, track client progress, and adjust care plans as needed to support long-term recovery and reintegration. Daily responsibilities include facilitating communication between clients, families, and the multidisciplinary team, organizing family meetings and follow-ups, and connecting clients with community resources and aftercare programs. The role also involves advocating for client needs, promoting a secure and supportive environment, and upholding ethical and confidentiality standards in all interactions.

Qualifications

- Strong case management skills, including assessment, care planning, coordination, and follow-up across multidisciplinary teams.




- Knowledge of mental health, addiction recovery, and psychosocial rehabilitation, with the ability to work compassionately with diverse client populations.
- Excellent communication and interpersonal skills for engaging with clients, families, and professionals, including conflict resolution and counseling support.
- Solid organizational and documentation abilities, including maintaining detailed records, using digital tools, and adhering to confidentiality and compliance standards.
- Bachelor’s or master’s degree in social work, Psychology, Counseling, or a related field; certification or training in addiction counseling or psychiatric social work is an advantage.
- Experience in residential rehabilitation, mental health services, or hospital-based care is highly beneficial.
- Ability to work on-site in the Greater Coimbatore Area, manage caseloads effectively, and collaborate within a multidisciplinary, evidence-based practice environment.
- Fluency in Hindi
