Job Description:
KEY CLINICAL RESPONSIBILITIES -
Patient Assessment: Perform initial and ongoing nursing assessments, including recording Vital Signs and pain scores.
Medication Administration: Safely administer medications (Oral, IV, IM) as per doctors orders, following the "7 Rights" of medication safety. The 7 Rights of Medication Administration-
Right Patient: Always verify the patient using two identifiers (e.g., Full Name and Hospital ID/UHID).
Right Drug: Check the medication label against the doctors order three times (before taking, while preparing, and before giving).
Right Dose: Double-check the dosage calculation, especially for "High-Alert" medications.
Right Route: Confirm if the medicine is to be given Oral, IV, IM, SC, etc.
Right Time: Ensure the medicine is given at the exact prescribed frequency (e.g., OD, BD, TDS) and timing.
Right Documentation: Record the administration immediately in the Medication Administration Record (MAR) with date, time, and signature.
Right Reason: Confirm that the medication is appropriate for the patients specific diagnosis/symptoms.
Nursing Procedures: Perform clinical tasks such as IV Cannulation, Catheterization, Dressing, and Ryles Tube insertion.
Care Planning:
Develop and implement individualized Nursing Care Plans based on patient needs and clinical status.
Monitoring: Identify and immediately report any change in patient condition (deterioration) to the treating consultant/Resident Doctor.
NABH & QUALITY COMPLIANCE -
Patient Safety: Strict adherence to International Patient Safety Goals (IPSG), including two-way patient identification and site marking.
Documentation: Maintain accurate, legible, and real-time nursing notes, Medication Administration Records (MAR), and Intake-Output charts.
Infection Control: Ensure 100% compliance with Hand Hygiene, use of Personal Protective Equipment (PPE), and proper BMW (Bio-Medical Waste) segregation.
Handover: Conduct structured clinical handovers using the SBAR (Situation, Background, Assessment, Recommendation) technique during shift changes.
COMPETENCIES AND SKILLS
Emergency Response: Proficiency in BLS (Basic Life Support) and handling of the Emergency Crash Cart.
Communication: Effective coordination with doctors, housekeeping, and patients' families to ensure Continuity of Care.
Ethics: Maintain patient privacy, dignity, and confidentiality at all times as per the Patients Rights and Responsibilities.
