The Staff Nurse will be responsible for the

A patient care coordinator's role in home health care is to manage a patient's healthcare and ensure their needs are met. Their responsibilities include:
- Assessments & Re-assessments: They will be responsible for doing the Clinical Assessments for the clients for preparing the care plans & provide the service to them.
- Care planning: Developing and implementing care plans for patients, based on their specific needs
- Patient education: Teaching patients about their health conditions
- Communication: Facilitating communication between patients, their families, and the Health Workers
- Coordination: Coordinating care from multiple providers and ensuring effective treatment plans
- Monitoring: Checking in on patients regularly and evaluating their progress
- Training:



Attending training and courses to stay up to date on healthcare developments and comply with health laws and regulations

Requirements and skills
- Proven work experience as a Registered Nursing Staff in Hospital.
- Good communication skills, and supervising skills
- Organizing skills
- Willing to stay in facility, can go home during week offs. Food & Accommodation will be provided conditions applied

Pay: ₹18,000.00 - ₹25,000.00 per month
