Role Purpose
To ensure seamless delivery of homecare services by managing patient assessments, coordinating clinician deployment, and maintaining high standards of clinical quality and compliance. The role acts as a bridge between patients, families, clinicians, and internal teams to ensure continuity of care, timely service delivery, and patient satisfaction.

Key Responsibilities

1. Patient Management & Assessment
- Receive and process patient inquiries, referrals, and service requests.
- Conduct initial patient assessments covering medical condition, personal needs, and care requirements through home visits or telephonic/video interactions.
- Collect, verify, and maintain patient documentation including medical history, prescriptions, and care plans.
- Educate patients and families about homecare services, care plans, scope of services, and processes.
- Address patient concerns, feedback, and escalations to ensure satisfaction throughout the care journey.

2. Clinician Management & Coordination
- Coordinate with nurses, caregivers, therapists,



and other clinicians for case assignments.
- Verify clinician credentials, experience, availability, and skill suitability for each patient case.
- Act as the primary point of contact for clinicians regarding assignments, schedules, and service requirements.
- Support onboarding, orientation, and deployment of clinicians to patient homes.

3. Placement & Deployment
- Match patients with appropriate clinicians based on care needs, clinical skills, location, and availability.
- Schedule and deploy clinicians for homecare services within defined timelines.
- Monitor deployment effectiveness and ensure continuity and quality of care.
- Manage replacements, escalations, and emergency deployments as required.

4. Documentation & Clinical Compliance
- Maintain accurate and up-to-date patient and clinician records in internal systems.
- Ensure compliance with organizational policies, clinical protocols, and healthcare regulations.
