Responsible for ensuring safe, effective, and rational use of medications in both inpatient and outpatient settings.
Key Roles and Responsibilities:
- Perform prescription audits in inpatient (IP) and outpatient (OP) departments.
- Provide medication counseling to patients to improve adherence and understanding of therapy.
- Conduct drug chart monitoring to ensure appropriate drug therapy.
- Recommend dose adjustments in renal and hepatic impairment.
- Monitor high-end and restricted antibiotics as part of antimicrobial stewardship.
- Review culture and sensitivity reports and optimize antibiotic therapy accordingly.
- Identify and remove short-expiry medications from wards, ICU, and OT.
- Conduct narcotic drug audits in critical care areas and ensure regulatory compliance.
- Monitor drug dilution procedures and compatibility during preparation and administration.
- Prepare chemotherapeutic admixtures and ion preparations under sterile conditions.
- Evaluate drug interactions and incompatibilities to prevent medication errors.
- Report Adverse Drug Reactions (ADR) through VigiFlow.
- Manage narcotic drug disposal as per institutional guidelines.
- Participate in Drug and Therapeutics Committee (DTC) meetings and presentations.
- Train nurses and healthcare staff on medication policies and safety protocols.
- Provide updates on recent drugs and treatment guidelines to physicians.
- Escalate drug recalls and stock-out situations to relevant healthcare professionals.
- Monitor vital drug availability in inpatient and outpatient areas.
- Prepare Antimicrobial Stewardship Program (AMSP) reports and monitor surgical prophylaxis practices.
- Collect feedback from physicians to improve pharmacy services.
- Maintain emergency crash carts and ensure removal of near-expiry medications.
Pay: ₹11,237.79 - ₹26,082.50 per month
