Job Summary

The Home Care Nursing Coordinator is responsible for coordinating, managing, and supervising all home nursing care services. The coordinator will ensure timely staff deployment, patient satisfaction, quality care delivery, staff scheduling, documentation, and effective communication between patients, families, nursing staff, hospitals, and management.
Patient Care Coordination
- Receive and process recent nursing service enquiries.
- Assess patient requirements and identify suitable nursing staff.
- Coordinate patient admissions and service commencement.
- Ensure continuity of care and proper handover between shifts.
- Monitor patient satisfaction and address concerns promptly.
- Conduct regular follow-up calls and home visits when required.

Staff Management
- Arrange and deploy qualified nurses and caregivers according to patient needs.
- Prepare and manage staff duty schedules and shift rosters.




- Handle emergency staff replacements during absences or emergencies.
- Monitor staff attendance, punctuality, and performance.
- Guide and support nursing staff in clinical and non-clinical matters.
- Conduct regular staff meetings and training sessions.

Quality Assurance
- Ensure all nursing services comply with company standards and clinical protocols.
- Monitor quality of care through home visits and patient feedback.
- Investigate and resolve service complaints.
- Implement corrective actions and continuous improvement initiatives.
- Maintain infection control and patient safety standards.

Pay: ₹30,000.00 - ₹50,000.00 per month
