Work Structure:
• Choose one shift:
8:30 AM to 2:30 PM (6 hours only)
3:30 PM to 9:30 PM (6 hours only)
• Sunday Off
Role:
• Minimum 5 home visits per day
• If fewer home visits must treat clinic patients during shift
• Proper assessment, treatment & documentation required
