Reporting To: Clinical Coordinator / Operations Manager

About the Role

We are looking for a compassionate, skilled, and professional Home Care Physiotherapist to provide physiotherapy services to patients in their homes.

The selected candidate will be responsible for assessing patients, developing individualized treatment plans, providing physiotherapy interventions, monitoring progress, and educating patients and caregivers on appropriate exercises and rehabilitation techniques.

Key Responsibilities

- Conduct comprehensive physiotherapy assessments at the patient's home.
- Evaluate mobility, strength, flexibility, balance, pain, and functional limitations.
- Develop individualized physiotherapy treatment and rehabilitation plans.
- Provide home-based physiotherapy treatments according to the patient's condition and clinical requirements.
- Provide therapeutic exercises, mobility training, strengthening exercises, stretching, balance training, and functional rehabilitation.
- Assist patients with post-operative, neurological, orthopedic, geriatric, and other rehabilitation needs.
- Provide appropriate gait and walking training using assistive devices when required.
- Educate patients and caregivers regarding home exercises, positioning, mobility, and fall prevention.
- Monitor patient progress and modify treatment plans when required.
- Maintain accurate patient assessment, treatment, and progress records.
- Communicate effectively with doctors, nurses, caregivers, and other healthcare professionals involved in the patient's care.
- Identify any deterioration or new clinical concerns and report them promptly to the appropriate healthcare professional.
- Follow infection-control, patient-safety, and professional standards during every home visit.




- Maintain patient confidentiality and respect the patient's privacy and dignity.
- Provide professional and compassionate care while working in the patient's home environment.

Patient Categories The physiotherapist may provide care for:
- Post-operative and orthopedic patients
- Stroke and neurological rehabilitation patients
- Elderly and mobility-impaired patients
- Patients recovering from fractures and injuries
- Patients requiring gait and balance training
- Patients with musculoskeletal conditions
- Patients requiring strengthening and functional rehabilitation
- Bedridden and long-term care patients
- Patients requiring rehabilitation following hospitalization

Requirements
- BPT / MPT from a recognized institution.
- Valid registration with the relevant professional authority, where applicable.
- Previous clinical or home-care experience is preferred.
- Good clinical assessment and physiotherapy treatment skills.
- Strong communication and interpersonal skills.
- Compassionate and patient-centered approach.
- Ability to work independently during home visits.
- Valuable documentation and reporting skills.
- Willingness to travel to patients' homes.
- Two-wheeler and valid driving licence will be an advantage.
