We are hiring a Health Care Coach for a patient support role involving post-discharge follow-ups, patient counselling, and engagement.

Note: This is not a sales or telecalling role.

Key Responsibilities
Follow up with patients after discharge to monitor health status
Provide emotional and psychosocial support to patients
Guide patients on recovery, medication, and lifestyle changes
Handle inbound patient calls and assist with onboarding
Maintain records of patient interactions

Eligibility Criteria (mandatory)
Bachelor’s degree in Clinical / Medical / Life Sciences / Biology:
Minimum 1 year experience in healthcare / patient care / counselling:
Good communication skills in English and Hindi:
Ability to speak clearly and handle patients with empathy
