Transcribe patient information: Convert patient information, such as physician's notes, lab reports, and diagnoses, into medical codes

Ensure accuracy: Verify the accuracy of medical records and codes

Follow guidelines: Adhere to medical coding guidelines and policies

Review documents: Examine medical records and documents for missing information or deficiencies

Audit charts: Conduct chart audits to ensure documentation supports diagnoses, procedures, and treatment results

Research and analyze: Research and analyze data needs for reimbursement, and identify discrepancies and billing issues

Assign codes: Assign codes for reimbursements, research, and compliance with regulatory requirements

Provide training: Orient, train, and mentor other coding staff

Analyze malpractice claims: Analyze medical malpractice claims to identify issues, events,



diagnoses, and procedures that led to the result

Prepare reports: Prepare summaries and assign the appropriate codes, and submit approved reports to managers
