Educational Qualification: GNM / B.Sc. Nursing/ MSc. Nursing from a recognized institution with valid registration with the respective State Nursing Council.

Experience: 3-7 years / 8-10 years

Role Overview: The DiNC Nurse will play a key role in supporting technology-enabled healthcare delivery through the Digital Nerve Centre. The role involves coordination with patient over tele-phonic/ video calls, preliminary clinical assessment, capturing vital information, supporting tele-consultations, patient counselling, follow-up coordination, digital documentation and facilitating continuity of care in accordance with approved protocols and guidelines.

Key Responsibilities

Ø Conduct preliminary patient assessment by collecting relevant medical history, presenting complaints and other available clinical information.

Ø Measure and accurately record patient vitals and other relevant clinical parameters, as required.

Ø Assist DiNC Doctors and specialists during telephone and video-based consultations.

Ø Provide basic health education and patient counselling on preventive care,



medication adherence, follow-ups and recommended lifestyle practices.

Ø Coordinate referrals, follow-ups, investigations and other recommended services as per the care plan.

Ø Conduct and support proactive follow-up of patients as per defined protocols, including reminders for consultations, investigations, medication and follow-up visits.

Ø Ensure accurate and timely documentation of patient information, clinical observations, consultation details, follow-up requirements and other relevant information on the designated digital platform.

Ø Follow TMDL's approved clinical protocols, SOPs, quality standards and patient-safety practices.
