Project Title:

“Integrated Community Health Program for NCD Screening (Hypertension and Diabetes) and Maternal-Newborn Care in Tea Garden and Forest-Fringe Villages of Kalchini Block, Alipurduar”

Project Overview:

To improve access to preventive healthcare, mass population screening, and primary care for ~21K people (out of total ~38K population) from ~7.6K households across 7 villages of 2 contiguous GPs in Kalchini block of Alipurduar. This includes screening and management of hypertension and diabetes (men and women aged 30 years and above), antenatal and postnatal care, and home-based newborn care and children below five years of age.

Role Summary:

The Project Manager will be based in Kalchini and hold overall responsibility for the day-to-day execution of the Integrated Community Health Program, operating under the strategic guidance and oversight of the Director. This role is the single point of accountability for translating the program design into functioning clinics, camps,



and community outreach on the ground.

Key Responsibilities

· Identify locations and lead establishment of recent clinic centres in the project area

· Organize pre-operative training for Community Health Workers and other field staff

· Complete pre-commissioning documentation for new centres

· Liaise with the Director on strategy, escalations, and progress reporting

· Liaise with local government health bodies and block-level health administration, including the Block Medical Officer of Health (BMOH), to secure referral pathways, priority access, and coordinated planning

· Supervise day-to-day functioning of all clusters — clinics, outreach camps, and door-to-door activity

· Verify staff attendance and maintain leave records across the team

· Oversee awareness/marketing activities to drive community uptake of services

· Prepare and submit monthly reports — both operational and financial — as per the Azim Premji Foundation-prescribed format

· Generate and submit daily/periodic reports to the Director and Head Office

· Monitor expenses and inventory (medicine stock, equipment) across all sites

· Review medicine stock indents, requisitions, and verification

· Coordinate with doctors (general practitioners and visiting specialists) on scheduling and case management

· Ensure the program stays aligned with government NCD/MCH guidelines and integrates with ASHA/ANM/AWW and VHSNC structures at the community level

Qualifications & Experience

· Master's degree in Social Work (MSW) or Master's in Public Health (MPH)

· Minimum 5 years of post-qualification experience, at least 3 years of which should be in a similar field

· Demonstrated ability to liaise effectively with local government health departments and block-level administration

· Strong people-management skills — the role directly supervises multiple layers of field staff

· Familiarity with donor/grant reporting requirements and financial tracking
