Patient Navigator / Care Coordinator – Kiran Program
About the Role
We are looking for a Patient Navigator / Care Coordinator to support patients throughout their therapy journey under the Kiran Program. The role involves patient coordination, documentation support, follow-ups, verification, and resolving documentation-related queries while ensuring a smooth and patient-centric experience.
The ideal candidate should have healthcare coordination experience, strong communication skills, empathy, and the ability to manage multiple patient requirements efficiently.
Roles & Responsibilities
- Provide Kiran-related documentation support to patients.
- Follow up with patients regarding pending Infusion Confirmation Forms (ICF).
- Connect with discontinued patients who have requested therapy re-initiation and support them through the required process.
- Resolve Kiran-related documentation queries raised by HCP Coordinators.
- Assist with patient verification and ensure required information is accurately captured.
- Coordinate with relevant internal teams and stakeholders to ensure timely completion of patient requirements.
- Maintain accurate and organized patient documentation and records.
Key Activities
1. Clinicians App
- Ensure appointments assigned through the Clinicians App are successfully completed.
- Track appointment status and coordinate with relevant stakeholders wherever required.
- Follow up on pending or incomplete appointments.
1. Patient Documentation
- Ensure all required patient documents are available, complete, and properly cross-checked.
- Identify missing, incomplete,
or incorrect documentation.
- Assist patients in completing and correcting documentation as required.
- Ensure documentation is processed accurately and within the required timelines.
Required Qualifications & Experience
- 1–2 years of experience in any of the following or similar healthcare roles:
- Patient Navigator
- Care Coordinator
- Hospital Key Account Manager
- Patient Relationship Executive
- Healthcare Coordinator
- Positive understanding of healthcare systems and patient-centered care.
- Strong communication and interpersonal skills.
- Ability to coordinate effectively with patients, healthcare professionals, and internal teams.
Preferred Qualifications
- Bilingual or multilingual communication skills will be an advantage.
- Prior on-field experience in healthcare or patient coordination will be preferred.
- Experience working directly with patients, hospitals, clinics, or healthcare professionals is an added advantage.

Ideal Candidate The ideal candidate will be a patient-focused healthcare professional who is comfortable interacting with patients, managing documentation, conducting follow-ups, and coordinating with multiple stakeholders. The candidate should be organized, empathetic, detail-oriented, and capable of resolving issues independently while maintaining a high standard of patient service.
Pay: ₹200,000.00 - ₹360,000.00 per year
