Key Responsibilities:
- Assess underlying factors related to chronic and lifestyle-associated health concerns
- Review laboratory investigations, diagnostic reports & medical records
- Develop individualized, evidence-informed health & wellness plans
- Guide patients on nutrition, exercise, sleep, stress management & healthy lifestyle practices
- Monitor patient progress and modify care plans as needed
- Coordinate with other healthcare professionals when required
- Educate patients on preventive healthcare and long-term health management
- Maintain accurate patient records and follow-up documentation
- Ensure patient confidentiality and clinical/ethical standards
