Conducting assesments of patient s social ,emotional ,and financial needs to develop care plans.
- Providing counselling and emotional support to patients and their families to help them cope with illness ,treatment, and recovery.
- Connecting patients with community resources such as support groups ,financial assistance programs.
- Advocating for patients rights and interests within the health care system ,ensuring they receive appropriate care & services.
- Offering education to patients and families about their medical conditions ,treatment options ,and available support services.
- Maintaining accurate and confidential records of assessment ,interventions and referrals in accordance with legal and ethical standards.




- Collaborating with interdisciplinary teams ,including Physicians ,nurses ,therapists and managers to coordinate comprehensive care of patients.
