The Clinical Pharmacist is responsible for providing comprehensive medication management, optimizing drug therapy, and ensuring strict adherence to hospital safety protocols. This role involves direct collaboration with physicians and nursing staff to elevate patient care standards and maintain full compliance with NABH accreditation requirements.
Key Responsibilities
- Clinical Rounds &
- Therapy Management:
Participate in daily multidisciplinary ward rounds. Review patient medication charts for appropriate dosing, therapeutic duplication, contraindications, and potential drug-drug or drug-food interactions.
- Medication Safety &
- Protocol Enforcement:
- Oversee the secure storage, dispensing, and auditing of Look-Alike Sound-Alike (LASA) drugs and High-Alert Medications.
- Enforce Independent Double Verification procedures for high-risk medications prior to administration.
- Ensure strict compliance with the Verbal Order Read Back (VORB) policy across all wards and intensive care units.
- Adverse Drug Reaction (ADR) Monitoring: Identify, document, and report any adverse drug events or medication errors to the Pharmacovigilance and Quality Committees.
- Patient Education &
- Counseling:
Counsel patients and their families upon admission and discharge regarding medication regimens, proper administration techniques, and expected side effects.
- NABH &
- Quality Assurance:
Support hospital-wide quality initiatives, conduct regular prescription audits, and assist in maintaining NABH standards for the Management of Medication (MOM) chapter.
- Drug Information: Act as the primary resource for evidence-based pharmaceutical information for the medical and nursing staff.
Qualifications &
- Experience
- Education: Pharm.D (Doctor of Pharmacy) or M.Pharm in Pharmacy Practice from an institution recognized by the Pharmacy Council of India.
- Licensure: Valid and active registration with the Kerala State Pharmacy Council.
- Experience: 1–3 years of clinical pharmacy experience in a multi-specialty hospital setting. (Fresh Pharm.D graduates with robust clinical knowledge may apply
Pay: ₹15,000.00 - ₹20,000.00 per month
