Description: Pana Community Hospital is seeking a skilled, compassionate, and patient-focused Nurse Practitioner (NP) to support our outreach clinics. The NP will provide comprehensive evaluation, diagnosis, treatment, and follow-up care in an outpatient setting while working collaboratively with physicians, nursing staff, and other healthcare specialists.

Responsibilities

Evaluate patients presenting for routine, preventive, acute, and chronic care needs in a clinic setting.
Obtain and document comprehensive patient histories and perform physical examinations.
Assess patient conditions and develop appropriate diagnoses and individualized treatment plans.
Order, perform, and interpret appropriate diagnostic testing, including laboratory studies, imaging,



and other tests within the Nurse Practitioner's scope of practice.
Diagnose and treat patients in accordance with established clinical guidelines, standards of care, applicable laws and regulations, and hospital policies.
Prescribe medications and treatments within the Nurse Practitioner's scope of licensure, applicable law, clinical privileges, and established practice requirements.
Perform office-based procedures and treatments consistent with the Nurse Practitioner's education, training, competency, scope of practice, and clinical privileges.
Educate patients and families regarding diagnoses, treatment plans, medications, follow-up care, and preventive health measures.
Recognize patients requiring a higher level of care and facilitate appropriate referral, transfer, or physician consultation when necessary.
Provide appropriate follow-up and continuity of care.
