We are seeking a qualified and dedicated Pharmacist (D.Pharm / B.Pharm) to join our hospital team. The Pharmacist will ensure secure, accurate, and effective medication management while supporting physicians and healthcare staff in delivering optimal patient care.
Key Responsibilities:
Review and verify medication orders for accuracy, dosage, contraindications, and drug interactions
Dispense medications in compliance with hospital policies and regulatory standards
Monitor patient drug therapy and provide clinical recommendations when necessary
Counsel patients and caregivers regarding medication use, dosage, side effects, and precautions
Collaborate with doctors, nurses, and other healthcare professionals to ensure protected treatment outcomes
Maintain accurate documentation and patient medication records
Oversee proper storage, labeling, and inventory management of medications, including controlled substances
Participate in medication safety and quality improvement initiatives
Requirements:
D.Pharm or B.Pharm degree from a recognized institution
Valid state pharmacy license/registration
Strong knowledge of pharmacology and clinical pharmacy practices
Valuable communication and interpersonal skills
Ability to work effectively in a hospital setting
Strong attention to detail and commitment to patient safety
