- Dispense prescription medications to patients and ensure proper dosage and administration
- Review and interpret physician orders to ensure accuracy and appropriateness
- Monitor patient responses to medications and adjust treatment plans as necessary
- Provide drug information and education to medical staff and patients
- Conduct medication reconciliation upon patient admission and discharge
- Manage inventory of pharmaceuticals and ensure proper storage conditions
- Collaborate with healthcare team to develop and implement individualized patient care plans
- Perform clinical rounds with physicians and other healthcare providers
- Participate in clinical research and trials as needed
- Supervise pharmacy technicians and other support staff
- Ensure compliance with regulatory requirements and hospital policies
- Maintain patient records and document medication therapy outcomes
Hospital Pharmacist Qualifications & Skills
- Advanced certification in pharmacy practice or clinical pharmacy
- Experience working in a hospital or acute care setting
- Experience working on MARG software
- Familiarity with hospital electronic health record (EHR) systems
- Strong analytical and problem-solving skills
- Excellent communication and interpersonal abilities
- Ability to work effectively in a quick-paced environment
- Commitment to continuous professional development and education
- Registered Pharmacist (RPh) license
